Operation AXIOM
10 April 1963: The Wreck of the U.S.S. Thresher SSN-593—The American submarine Thresher sank in the Atlantic Ocean during dive tests, with one hundred twenty-nine souls aboard; the Thresher had been circling the support ship U.S.S. Skylark; debris was first detected on 12 April & the wreck was found in June 1964; the Navy's Submarine Safety Program (SUBSAFE) was instituted as a response.
Commentary: The Thresher was not one of the Submarines of 1968, she was lost five years earlier & sank while in communication with the Skylark rather than disappearing mysteriously.
