Charlie Duke aboard the Command & Service Module Casper atop a Saturn V rocket also carrying the Lunar Module Orion; a potential fuel leak from the Orion, observed during extraction from the S-IVB, would have aborted the Moon landing, but turned out to be peeling paint.Bonus!
